SOURCING ORDER FORM

Brief Purchase Description:   provide a brief description of the items to be ordered e.g. Apparel, Shoes and Electronics from Amazon.com and BestBuy.com.  After submitting your order, send additional details on the items to be ordered using one of the formats specified below.

Total Purchase Value:  this must include the total cost of the items to be purchased as well as any associated taxes and shipping costs.

Estimated Weight (in Kg):  this is usually provided on the merchant site for each item. If not, your best estimate if fine. The actual weight will be charged on your final invoice. The minimum weight is 1kg.  2.2lb = 1kg

Number of Stores:  this is the # of stores you want to order from

Attach Order Details: you can also attach your order details in one of the formats below  (please do not send pdf files):

  1. Put the items in the cart on the merchant website, copy and paste the cart in MS Word and send as an attachment.
  2. Copy and paste the web site link of each item to be purchased in an MS Word document, specify the price, quantity, size, color, etc. as applicable and send as an attachment or in the body of an email.
  3. Copy and paste the web site link of each item to be purchased in an MS Excel spreadsheet, specify the price, quantity, size, color, etc. as applicable and send as an attachment.
  4. Put each item in the cart on the merchant’s website and send us your login details.

 All other fields will be calculated.

PAYMENT ORDER FORM

Payment Description:   provide a brief description of the payment you want to make and how the payment is to be made (via a web site, by mail, through PayPal, Wire Transfer, etc.).  Where login details are required to access your account to make the payment or the name, email address, mailing address, phone # and other details of the recipient is required, please include in this field.

You can also include multiple payments in a single order form.  Include the description for each payment in this field and enter the overall total payment amount in the field below.

Payment Amount:   enter the amount to be paid in this field.  Ensure that any additional fees involved in making the payment are included e.g. wire fees, western union fees, etc.  If you do not know the additional fee involved, include only the payment amount and indicate under the description field above how the payment is to be made (i.e. wire transfer, western union, etc.).  We will send you a revised quote including the additional fees.

All other fields will be calculated.

After completing and submitting the order form, follow the instructions on the email you receive.

DEBIT CARD ORDER FORM

Debit Card Amount:   select the Debit Card value you want to purchase from the drop down list.  These are the only card values that we offer.

Additional Information:   enter any additional information you want to communicate.  If you require multiple debit cards at the same time, you can also include this information in this field and we will contact you regarding applicable discounts.

All other fields will be calculated.

After completing and submitting the order form, follow the instructions on the email you receive.
